Physical Clearance
Sports physicals must be current for the entire season. Once a physical is on file, it is valid for 2 years from the date it is signed by the medical provider. You can download forms at https://www.osaa.org/docs/forms/Physical%20Examination%20Form%20(EN%20r2017).pdf  Physical Forms are available in Spanish, Chinese, Vietnamese, and Russian here.

Academic Eligibility
All Freshman begin with academic eligibility for the Fall Season. For Sophomores, Juniors and Seniors, eligibility for sports participation is based on GPA from the previous semester. Eligibility for all grades is then based on 1st quarter grades for the Winter season and 1st semester grades for the Spring season. For more information, check
www.pilathletics.com or www.osaa.org.

Athletic fees can be paid online at www.schoolpay.com.  You can also submit a payment by cash, check or card in our bookkeeper’s office. PIL sponsored sports are $200 per sport per student.  For students on the Federal Free/Reduced Lunch program, the athletic fee is $35 per sport.  The PIL wants all students to be able to participate in a sport if eligible. If needed, please contact the Athletic Department at Cleveland to discuss athletic fees.
