Athletic Clearance


- Preseason Grade Check (Winter Season)
Students must turn in a preseason grade check form or a printed Synergy Report Card at the beginning of Winter Season ONLY. This must be dated within 2 weeks of the start of the season. Turn into athletics office with blue card. If you are turning in a printed copy of your Synergy Report Card you must have a grade showing in every class (no N/A). If you have classes with N/A listed you must obtain a signature from that teacher with your current grade.

-Physical / Physical Update Form    
A student participating in athletics must have a physical examination by a medical practitioner (valid for 2 years). Students who have a valid physical on file need to complete an Update Form on the years between sports physicals. If athletes have turned in a Physical or Update form for a fall sport, they will not need to do so again for the Winter or Spring seasons.  Links to these forms can be found below. The Cleveland Health Center offers sports physicals.

- Blue Card
Every athlete needs to have a Blue Card filled out and signed by parents/guardian and turned in to the Athletic Office prior to participation.  This card will be given to your coach when you are cleared for participation. They can be found in the Main Office or Athletic Office. 

To be cleared academically:                                       
1.Student must have passed at least 5 course last semester
2. Be currently enrolled and passing at least 5 courses this semester
3.Earned the following number of credits:
-4.0 credits by start of 10th grade
-9.5 credits by start of 11th grade
-16 credits by start of 12th grade
4.Earned the following minimum GPA last semester:
- 2.0 GPA with no F's
- 2.5 GPA with one F

FAILURE TO MEET (4) = ACADEMIC PROBATION ***academic probation for two consecutive grading periods will make athlete ineligible until the next grading period (9 wks). 


PIL Sponsored Sports

- $200 per sport per student. 
 There is a two fee cap per student.  (i.e. if a student plays a fall and winter PIL sport, the spring sport fee is waived)

-There is a four fee family cap per year per family.

-For students on the Federal Free/Reduced Lunch program, the athletic fee is $35 per sport.

The PIL WANTS all students to be able to participate in a sport if eligible. If needed, please contact the Athletic Department at Cleveland to discuss athletic fees.
