To make an appointment call 503-988-3350 - During summer the Cleveland phone number will forward to Parkrose. The Parkrose Clinic can schedule an appointment at Cleveland Health Center.
Students can also obtain FREE sports physicals all summer at Parkrose High School.

Physicals need to be complete and uploaded to student registrations prior to August 15 for athletes wishing to complete in Fall Sports. Students can not actively participate in tryouts or practice until they are FULLY cleared by the athletic department.
